THYROGLOSSAL CYST EXCISION - THYROGLOSSAL CYST- SISTRUNK OPERATION A thyroglossal duct cyst is a mass or lump in the front part of neck that is filled with fluid. While a baby is developing in the womb, the thyroid gland begins at the base of the tongue. Before birth the thyroid gland moves in the neck to its usual position below the thyroid cartilage and above the sternum Thyroglossal duct cysts typically present as mobile midline neck masses near the hyoid bone. They often are asymptomatic. However, they can present as an abscess or intermittently draining sinus. The mass will elevate with tongue protrusion or swallowing. The mass is closely associated with the hyoid bone and most commonly found at or below the level of the hyoid. The treatment for thyroglossal duct cysts is surgical removal to prevent recurrent infections due to the small risk of malignancy. Simple excision of thyroglossal duct cysts is associated with high recurrence rates (45% to 55%). The Sistrunk operation is considered the standard of surgical management and has dramatically reduced recurrence rates. This procedure requires a more extensive surgical resection including the central third of the hyoid bone and a core of base of tongue tissue.
VAGINAL HYSTERECTOMY - VAGINAL HYSTERECTOMY Hysterectomy is one of the most frequently performed surgical procedures. A hysterectomy is the surgical removal of the uterus, and most likely, the cervix. The most common reason for a hysterectomy is uterine fibroids. A vaginal hysterectomy is a procedure to remove the uterus. Most people can go home within 24 hours of surgery, but it often takes 6 weeks to return to daily activities. GANGRENOUS PERFORATED GALL BLADDER -LAPAROSCOPIC CHOLECYSTECTOMY - Gangrenous cholecystitis is a form of acute cholecystitis which involves gangrenous alterations in the gallbladder wall and it often follows an acute and serious course. 64 year old lady presented with severe pain in right hypochondrium and vomiting to Archana Multispeciality Hospital, Tirupati. Dr. Archana examined her and diagnosed as Acute Cholecystitis and Laparoscopic Cholecystectomy was done. Intraoperatively it was gangrenous gall bladder, surgery went on successfully and patient was discharged on 3rd Postoperative day. Gangrenous cholecystitis is defined as cholecystitis involving necrosis and the bleeding of tissues after edematous alterations. Early and appropriate treatment is desirable; however, making a preoperative diagnosis is often difficult. However, for cholecystitis with severe local inflammation such as gangrenous cholecystitis, it is often difficult to perform LC perfectly according to the above procedure. In the above case, it is recommended to consider using laparosonic coagulating shears, adding ports, the fundus first technique, subtotal cholecystectomy and open conversion. If these also present with challenges, then we perform fundus first technique. Although a definite diagnosis of gangrenous cholecystitis is difficult to make prior to surgery, if an early diagnosis can be made and appropriate treatment methods can be carried out,then even very elderly people may be discharged without any major complications.
