ANAL FISSURE -  Anal Fissure The most common symptoms of anal fissures are:  a sharp pain when you poo, often followed by a deep burning pain that may last several hours bleeding when you poo – most people notice a small amount of bright red blood either in their poo or on the toilet paper. Most cases occur in people who have constipation, when a particularly hard or large poo tears the lining of the anal canal. Self-help measures for avoiding constipation include:  plenty of fibre in your diet, such as fruit and vegetables and wholemeal bread, pasta and rice – adults should aim to eat at least 30g of fibre a day staying well hydrated by drinking plenty of fluids not ignoring the urge to poo – this can cause your poo to dry out and become harder to pass exercising regularly – you should aim to do at least 150 minutes of physical activity every week Surgery Surgery may be recommended if other treatments have not worked.   It is generally considered to be the most effective treatment for anal fissures, with more than 9 out of 10 of people experiencing good long-term results. 

LIPOMA -  LIPOMA A lipoma is a lump of fatty tissue that grows just under the skin. Lipomas move easily when you touch them and feel rubbery, not hard.  Lipomas can appear anywhere on the body, but they’re most common on the back, trunk (torso), arms, shoulders and neck. Lipoma removal procedures are safe and effective, and you can usually go home the same day. 

LAPAROSCOPIC APPENDICECTOMY - LAPAROSCOPY APPENDICECTOMY An appendicectomy is an operation to remove the appendix, which is a small, tube-like part of the bowel. It often needs to be performed urgently when someone has an infected and inflamed appendix (appendicitis). It is also known as an appendicectomy. An appendicectomy is usually done because the appendix is inflamed or has burst. A laparoscopic (lap-a-ro- SKOPP-ik) or “lap” appendectomy is a minimally invasive surgery to remove the appendix through several small incisions, rather than through one large one. Recovery time from the lap appendectomy is short. Advantages of laparoscopic appendectomy The results may vary depending upon the type of surgery and your general health. But the most common advantages of laparoscopic surgery are:  Less pain after surgery. A shorter time in the hospital. Getting back to normal activity faster Having normal bowel movements sooner. A smaller scar. 

LAPAROSCOPIC HYSTERECTOMY -  LAPAROSCOPIC HYSTERECTOMY A laparoscopic hysterectomy is a minimally invasive surgical procedure to remove the uterus.  Abnormal or heavy vaginal bleeding that isn’t managed by other treatment methods. Severe pain with menses that isn’t managed by other treatment methods Leiomyomas or uterine fibroids (noncancerous tumors). Increased pelvic pain related to your uterus but not managed by other treatments. Uterine prolapse (uterus that has “dropped” into your vaginal canal due to weakened support muscles) that can lead to urinary incontinence or difficulty with bowel movements. Cervical or uterine cancer or abnormalities that may lead to cancer for cancer prevention. Conditions with the lining of your uterus, like hyperplasia, recurrent uterine polyps or adenomyosis. LAPAROSCOPIC HYSTERECTOMY is a minimally invasive procedure that is designed to replace abdominal hysterectomy. It has been well established that patients who have a laparoscopic hysterectomy have less pain, less bleeding, less risk of infection and are quicker to return to work and normal activities than women who have an abdominal hysterectomy. THYROGLOSSAL CYST EXCISION -  THYROGLOSSAL CYST- SISTRUNK OPERATION A thyroglossal duct cyst is a mass or lump in the front part of neck that is filled with fluid. While a baby is developing in the womb, the thyroid gland begins at the base of the tongue. Before birth the thyroid gland moves in the neck to its usual position below the thyroid cartilage and above the sternum Thyroglossal duct cysts typically present as mobile midline neck masses near the hyoid bone. They often are asymptomatic. However, they can present as an abscess or intermittently draining sinus. The mass will elevate with tongue protrusion or swallowing. The mass is closely associated with the hyoid bone and most commonly found at or below the level of the hyoid. The treatment for thyroglossal duct cysts is surgical removal to prevent recurrent infections due to the small risk of malignancy. Simple excision of thyroglossal duct cysts is associated with high recurrence rates (45% to 55%). The Sistrunk operation is considered the standard of surgical management and has dramatically reduced recurrence rates. This procedure requires a more extensive surgical resection including the central third of the hyoid bone and a core of base of tongue tissue. 

VAGINAL HYSTERECTOMY -  VAGINAL HYSTERECTOMY  Hysterectomy is one of the most frequently performed surgical procedures. A hysterectomy is the surgical removal of the uterus, and most likely, the cervix. The most common reason for a hysterectomy is uterine fibroids. Others reasons include: • Endometriosis, which is an overgrowth of tissue in the uterine lining • Adenomyosis, which occurs when endometrial tissue grows into the walls of the uterus • uterine prolapse • Abnormal bleeding • Chronic pelvic pain • Gynecologic cancer or precancer There are three ways to perform a hysterectomy: • Abdominally, through a surgical incision • Laparoscopically • Vaginally. Vaginal hysterectomy • Your uterus is removed through an incision at the top of your vagina. There isn’t an external incision. • Dissolvable stitches are placed inside your vagina. • Most commonly used in cases of uterine prolapse and other nonmalignant (or noncancerous) conditions. • Fewest complications and fastest recovery (up to four weeks) and considered the preferred approach. • People often go home on the same day of surgery. A vaginal hysterectomy is a procedure to remove the uterus. Most people can go home within 24 hours of surgery, but it often takes 6 weeks to return to daily activities. There may also be long-term hormonal effects. 

GANGRENOUS PERFORATED GALL BLADDER -LAPAROSCOPIC CHOLECYSTECTOMY -  Gangrenous cholecystitis is a form of acute cholecystitis which involves gangrenous alterations in the gallbladder wall and it often follows an acute and serious course. 64 year old lady presented with severe pain in right hypochondrium and vomiting to Archana Multispeciality Hospital, Tirupati. Dr. Archana examined her and diagnosed as Acute Cholecystitis and Laparoscopic Cholecystectomy was done. Intraoperatively it was gangrenous gall bladder, surgery went on successfully and patient was discharged on 3rd Postoperative day. Gangrenous cholecystitis is defined as cholecystitis involving necrosis and the bleeding of tissues after edematous alterations. Early and appropriate treatment is desirable; however, making a preoperative diagnosis is often difficult. However, for cholecystitis with severe local inflammation such as gangrenous cholecystitis, it is often difficult to perform LC perfectly according to the above procedure. In the above case, it is recommended to consider using laparosonic coagulating shears, adding ports, the fundus first technique, subtotal cholecystectomy and open conversion. If these also present with challenges, then we perform fundus first technique. Although a definite diagnosis of gangrenous cholecystitis is difficult to make prior to surgery, if an early diagnosis can be made and appropriate treatment methods can be carried out,then even very elderly people may be discharged without any major complications.
